When Programming a Key and or Electronic Control Modules that require Immobilizer codes to be entered. We will require positive ID verification performed by the shop unless the vehicle owner is on-site at the time of our arrival. 

Items we require when performing immobilizer operations:

• Copy of vehicle owner's Driver's License

• Copy of vehicle Registration or Title 

• Copy of Signed D-1 Form fill out the top 2 sections only 

• Phone number in which to contact the vehicle owner

This information is solely used to verify the identity of ownership and permission to program keys or perform immobilizer functions when replacing control modules in a vehicle. Information will be uploaded to the National Automotive Safety Task Force.
